I think if I were to do it again, I'd have a Z-plasty and go to Dr Alter. My scrotum was left looking deformed. It looks fine when the penis is warm and relaxed, but when it tightens up due to weather or adrenalin etc, the scar contracts and it looks really odd. I was embarrassed by the appearance. At the end of the video where Dr Carrion shows the bottom of the incision and it sticks out slightly, that becomes very apparent when the scrotal skin is tight and it looks like a lump of skin is sticking out from the scrotum. Go to 3.02-3.05 mins on the video and you'll see the flap of skin I'm talking about. Also consider that in the clip the scar tissue hasn't formed, which again effects the shape to the scrotum. I had 2 surgeries to fix mine and it still doesn't look great. It is my opinion that any surgery that one needs to be done to his penis, it should be done waaaaay before PMMA....I think PMMA should be done once you have achieved your goals and healed completely and then give it 3-6 months of rest with tons of sex then go for PMMA.
