This is not meant to be a pile-on or a callout. It’s a patient-safety post. **What we’re seeing in these Repair cases** Many of these men describe some version of: - A **rapid escalation** in total PMMA volume over a short period of time (sometimes in a “go big early” plan). - New or worsening **pain, burning, or deep tenderness** that persists. - **Inflammatory swelling** that doesn’t fully calm down. - Firm nodules or areas that later get labeled as **granuloma** (sometimes months after the injections). - A long “wait and see” period where they’re told it’s normal—until it clearly isn’t. To be clear: PMMA complications can occur even with careful technique, because PMMA is a permanent particulate filler and the body’s response can be unpredictable. But in our experience, the risk profile changes dramatically when men receive **too much filler too fast**, without adequate time for tissue response to declare itself. **The “deeper injection technique = no complications” claim is misleading** We also keep hearing the same marketing message from patients: that their provider promotes a “deeper” injection plane as inherently safer—sometimes even implying that it means **no complications**. That’s simply not how biology works. **Depth does not equal immunity.** No injection “plane” magically eliminates: - inflammatory reactions, - granuloma formation, - infection or biofilm risk, - vascular/lymphatic irritation, - nerve irritation, - or delayed inflammatory responses that show up months later. A deeper technique may change which complications are more likely, or how they present, but it does **not** mean complications can’t happen—and it definitely does not justify a high-volume, high-speed approach. **Why “too much, too soon” can be a recipe for trouble** When PMMA is placed, the goal is a controlled, stable tissue response. When the tissue is overwhelmed—by **volume, density, repeat sessions too close together, or aggressive placement**—the body can respond with chronic inflammation. That inflammation can become self-sustaining, and in some men, it can evolve into painful nodularity or granulomatous reactions. We’re not saying every high-volume case will fail. We are saying that if you’re told “more is better” or “we can do it fast” or “our technique prevents complications,” you should treat that as a red flag and get another opinion. **What to do if you already had PMMA and you’re worried** If you’ve had PMMA and you’re experiencing any of the following, don’t ignore it: - Pain that persists beyond the early healing window or is worsening - Burning, throbbing, deep tenderness, or pain with erections - Persistent swelling, firmness, or heat - New lumps that feel “fixed” or progressively harder - Skin changes, drainage, ulceration, or fever (urgent evaluation) Early, appropriate evaluation can matter. Some issues are manageable when addressed promptly, and much harder when they’ve been smoldering for months. **Practical takeaways before anyone gets PMMA** If you’re considering PMMA (or already planning it), here are the safety principles we’d encourage: 1. **Stage conservatively.** Give your body time to respond before adding more permanent material. 2. **Be skeptical of promises.** “No complications” claims are marketing, not medicine. 3. **Ask about complication management.** What’s the plan if you develop pain, nodules, or granuloma? 4. **Ask about follow-up access.** Permanent filler requires long-term accountability. 5. **Get a second opinion** if your plan feels rushed or volume-heavy. Unfortunately, it not only depends on starting size but also penile anatomy and tissue characteristics, so I really can't give you any kind of generalization because every man truly is different. After we have the foundational layers of HA in place and are ready to introduce Bellafill, it can be anywhere from 5 syringes (or even less if appropriate) to 8 syringes per session, again depending on what is medically appropriate for the patient. And that's not at all to say that every patient who undergoes girth enhancement with PMMA will face complications. However, to be completely honest and transparent with you and everyone else on this site, many of these patients are coming to us after having gone to a well-known injector for their PMMA. I will leave you to speculate as to why these patients have not spoken up. And you're very welcome. As always, we are first and foremost a urology and men's sexual health clinic and always have been. We just happened to find that the same safe, urology-focused techniques we used to help our Peyronie's patients regain the size and contour they lost to the disease also work quite well for patients without PD who just want a bigger penis.
