I'm fond of the noose. I just use the silicone tube tightened and tucked behind the glans. Works really good and has given my glans a ridge that makes it look larger. I've never had luck with the velcro or rubber belt style straps that come with extenders, just the noose. Sweat pants are ideal. Jeans suck to try and extend in. I wear mind flipped up or pointed down. I mix up the different angles depending on clothing. If you have a labor intensive job or physical activity, wearing an extender isn't possible. But just cruising down the sidewalk in sweats, nobody will notice. Extending can seem to enhance the "turkey neck" (at least for me) and does enhance skin growth a lot. I have lots of extra skin on my Dick now from extending. Seems like a good thing from what I've heard with PMMA though when I want to get my op. Tight skin seems to be troublesome. I wish I would have started extending in high school when I didn't have shit to do all day but sit down at a desk. It stretches sideways (either side) with the glans attached to a belt that goes round the waist. Great for using under clothing. Attachment is by noose though ...
