I think guys on here who have had the procedure done can really offer you a lot of advice, but I’m going to offer advice from a treating physician perspective. First, if you’re concerned about safety or the procedure itself, I would strongly consider starting with a hyaluronic acid (HA) filler. These are very long-lasting—you can expect two to three years of benefit—and they are 100% reversible. They are also extremely effective. I’ve treated many patients, including close friends, and the satisfaction level is consistently very high. You didn’t mention where you’re based, but in the U.S. the only permanent filler is PMMA (Bellafill). It has been FDA-approved for nearly two decades. For penile girth enhancement, it is used off-label, which is an important distinction to understand. There are drawbacks to permanent fillers, and that’s a conversation you should have in detail with any provider you’re considering. In my opinion, the primary drawback of PMMA is the risk of granulomas. That risk is low, but it is not zero, and it is inherent to the nature of PMMA. There are ways to reduce the risk, but it cannot be eliminated entirely. In the clinical trials of Bellafill the chance of a granuloma was about 1/20. My final advice: don’t wait years if this is something you’re seriously considering. One of the most common things I hear from patients after treatment is that they wish they had done it sooner. I think this supposedly lowers the risk of granulomas but i’ve yet to do more looking about it. What I actually prefer to do is inject a mix of HA and PMMA at the same time. I believe the HA slows the exposure of the PMMA to the immune system and may reduce the risk of granuloma formation. In addition, the combination produces a very natural-looking result.
