A gradual approach allows for precise fine-tuning and optimal aesthetics, but we have also had many “one-and-done” cases with highly satisfactory outcomes. Circumcision is not a requirement in our practice. In our experience, PMMA becomes incorporated into the expanded dermis, and the dermis itself does not migrate—even with friction or force—so long-term filler displacement into the Foreskin is not a concern. What may be happening is that in pts with very large volumes the lymphatics are disrupted and over time you can get swelling in the Foreskin this is not migration. I have only seen this in men howget very large volumes. With proper planning and technique, uncircumcised patients can achieve a smooth, natural-looking result.
