Seems quite precarious. How is it that the clinician 'knows' with any degree of confidence that he's entered the correct depth & cavity and hasn't incorrectly overshot/undershot? Precision is ultra important for any filler (perhaps less so for a bio-identical temporary HA-type filler) but when speaking of a PMMA-type product specifically -- the PMMA spheres (even if a tiny amount) will forever be in the wrong place -- creating a cascade of unintended consequences. I would love insight on how William Moore came to know this, but they can’t give away all their secrets. What I know my doctor has told me is that he can “feel it”. Just like if you put styroam layers atop each other and had each have a different thickness you might notice passing through the layers with your needle. I imagine the corpus caverns has a distinct feel from the rest of the penis, and the different layers so. If your physician CANT answer this then I suggest you RUN. Coming from someone who’s had 12cc of filler botched, and then 15cc of filler botched. Conventionally it's between the dartos and **buck's fascia** and isn't precarious at all. In fact the technique is remarkably efficacious, as demonstrated by an overwhelming authentic patient reporting of filler injections in general. You'll find many on this site, but they have been sporadically mentioned on other PE sites as well. What's more *"precarious"* is choosing the wrong practitioner and/or filler type. There are some bad actors out there who opt to inject non-FDA approved filler cocktails (often containing silicone oil), and others opting for the use of rigid silicone implants for the purposes of augmentation (and not erectile dysfunction). And there are a bunch of "Botox Mills" who have access to the same fillers off-label who may not understand the critical technique & methodology of injecting the filler correctly (let alone aesthetically), so your concerns are valid as someone approaching this scene. I've seen some of the work through back channels (PMs + Emails) and bad quality results very much exists and persists. I'm here to tell you that (fortunately), there are leading Docs in the field and they are excellent at what they do. The previous poster mentioned PhalloFILL, their [ Dallas Clinic](https://phalloboards.info/directory/85-phallofill.html) being a great option for Hyaluronic Acid (HA). Other popular fillers include PMMA (a permanent option), and [ Jiva Med Spa](https://phalloboards.info/directory/90-dr-rakesh-nanda.html) has put out some quality work in Ohio, and [ Dr. Luis Lee](https://phalloboards.info/directory/97-dr--luis-lee-northern-california.html) out of the Bay Area offers a bit of everything (P-Long, P-Shot, Girth Injections, etc.). While the industry has a poor history of botcheries and has largely remained experimental (i.e. lacking standardization), I would contend that injectable phalloplasty has really come a long way in the past 10 years and we've entered the first ever *Golden Age* of medical male enhancement in the 2020's. He's not just well reviewed for male enhancement, but has an overwhelming number of positive reviews with female aesthetics (like the face), which comes with a higher degree of skill to ensure the most seen part(s) of a woman retains as much naturalness as possible while enhancing it for symmetry & proportion. Each area of treatment, i would assume, take a slightly different approach, preference of filler, and skillful hands to achieve. Most of the doctors who successfully do this on a regular basis (not once in a blue moon kind of procedure) know this space well and it is a very thin space but the horizontal plane of this space is quite vast (continuous along the entire length of the shaft). A good analogy is when you get into your bed, there is the comforter, the sheet below the comforter, and the sheet that wraps the mattress. The space between Dartos and Bucks fascia is in between the latter two sheets. After you climb into your bed enough times you know how to snuggle exactly in between those two sheets. In the face we have these "spaces" in several areas and then the cannula is in that space has a certain "smoothness" when the cannula is gliding in this space or plane. Finally the appearance of the filler will be totally different if injected just into the "comforter" layer which would be more lumpy or irregular. Hope this helps. My doctor did not do this when I got my PRP shot in the Spring 2025. Everything went fine but the use of ultra sounds is one thing I’m looking at for my next PRP shot and my filler injection, in the future. It’s not a deal breaker in getting either done but is just another thing to consider. I do have a very sophosticated ultrasound system in my office but never feel the need to use it for PRP or Exosome/stem cell injection into the corpora. I could market that I have one but don’t see the benefit if the doctor is skilled enough. I’m not at all old school. Bc I actually invested in an ultrasound. Just don’t think it’s necessary or practical for certain procedures. This is just my opinion. (And others)….Can you address the methods and best practices for numbing the penis for receiving the PRP shots? The doctor that performed my initial shot applied numbing cream above and on my penis and then applied three shots above the penis. After 20 minutes the PRP shits were applied and I did not feel anything. This is in contrast to where I have read that some doctors on apply numbing cream to the penis. It seems this could be a less painless method. Majority of my patients tolerate PRP or exosome injections with only really strong numbing cream and rate it a 1/10 pain. I even had the Pshot done on me by Dr. Runels who invented the procedure and it was a 1/10 pain level. Most of the trepidation/fear is anticipation. But I totally understand if you’ve never done it before having a needle inspected into your penis can be frightening. In fact, it's like whenever you get any sort of needle in your life (blood drawn, vaccinations, etc.) -- the initial anticipation of that pain gets you worked up, but after it's all said & done, you look back on it and go, *"meh, that wasn't so bad."* And to be frank, as someone who has grown up being embarrassingly scared of needles, these procedures actually played a big part in me getting over my fear of shots and prickly things -- the local anesthetics work like a charm, the initial pinch is a walk in the park really.
