A lot are apprehensive to cut it perfectly tight. I wouldn't ask for "loose" though. Maybe just tell them you don't want it too loose or too tight. The thing is, a good majority of doctors aren't practicing cosmetic circumcisions and won't really listen to your request on where to put the scar line. They do it for a medical purpose. You can find some that will customize it more towards your wants though. Dr. Horn has been doing these for quite some time and the approach has been endorsed by Rejuvall in the States as well (not sure if they got the idea from Androfill or if they too realized the benefits of an initial introduction of HA to your penis in low volumes before subsequent PMMA fillers). I'm not sure this newer methodology is necessary per-se, but I can see its benefits, especially from the practitioner's point-of-view (i.e. maximizing the chances of a smooth result). I've heard of some doctors inducing erections during the procedure so they can better see what it will look like Erect but this can have several issues so it's not normally done. Even just making it a little tighter than it should be will negatively impact your Erection, i.e. making it shorter. It's difficult even for skilled and experienced surgeons to determine exactly how much skin should be removed. So in general, they error on the side of caution and leave it a little loose. Like I've said several times, the average Urologist at your local hospital won't listen to your aesthetic wishes. They aren't trained to do that. But in general, Circumcision has been found to cause no loss in length. The doctor is quite well known on here, but I’m not sure if I should be dropping names. I was told waiting too long makes it harder to get the cannula in the right areas, if needed, without risk of pushing the collagen/fibrous tissue up, thus creating raised areas etc. I’m likely butchering it as I’m just a simple fella, but that’s the gist I got from what I was told.
